I'm just another Yank who has a real difficulty distinguishing between the two. There are times I can tell the difference, but this wasn't it.

It's becoming much more widespread. For a time, a year or so back, it was everywhere, but, thankfully, there's been a bit of a revolt against it, so with any luck we've nipped it in the bud.

I'm not saying there's only one "standard British accent," Peter, but his would be one of the most common, to say the least.
